We are participating providers on the plans listed below. Dr. Martinez is a contracted practitioner with most major insurance panels. If we are not participating providers, out of network benefits may be pursued. Reduced fees are available on a limited basis.
No Show Fee
The patient understands there is a charge of $78.00 that will be required for any appointment not cancelled 24 hours in advance. You may call the office 305-826-6969 or email gofficephd@gmail.com in order to cancel.
